PSYCHOLOGICAL FEATURES OF EXPERIENCING GUILT AND SHAME IN CANCER PATIENTS IN THE CONTEXT OF PSYCHOCORRECTIONAL WORK

The article presents a theoretical analysis of the psychological features of guilt and shame in cancer patients and outlines the main directions of psychological interventions aimed at addressing these experiences. Guilt and shame are conceptualized as clinically significant components of the psycho-oncological experience that mediate the patient’s psychological state, adaptation to illness, attitude toward treatment, interpersonal functioning, coping behavior and readiness to seek psychological help. It has been established that, in the context of cancer, guilt is predominantly associated with the experience of responsibility for possible causes of the disease, delayed seeking of medical care, insufficient care for one’s own health, and the feeling of being a burden to close others. Shame is associated with global negative self-evaluation, body image disturbance, loss of autonomy, social vulnerability, and the experience of stigmatization. It is shown that guilt is related to the evaluation of action, inaction, and moral responsibility, whereas shame is integrated into the overall self-concept and is accompanied by feelings of defectiveness, undesirability, selfisolation, and distress. The main targets of psychological interventions have been identified as excessive self-blame, global negative self-evaluation, self-criticism, rumination, bodily stigmatization, avoidant behavior, body image disturbance, and dysfunctional family-related meanings. The relevance of applying the cognitive-behavioral approach, compassion-focused therapy, acceptance and commitment therapy, psychoeducation, supportive counseling, selfforgiveness interventions and family-oriented support is outlined
